  • Blended Learning: COVID-19 Consent Form

    For BLENDED students only (Grades 1-5): The following forms must be brought in to school on the first day students return to school in person, Tuesday, December 8th. Submitting consent to have your child tested for COVID-19 in school is quick and easy. Even if you have already submitted consent, we ask you to do so again to ensure your student has the latest consent form on file. There are two easy ways to submit:

    1) Fill out the form online using a New York City Schools Account (NYCSA) at
    2) Print and sign the attached form and bring it to your child’s school on the first day they are back in the school building, Tuesday, December 8th. A printable PDF file is available in ten languages

    Dear Families: Even though breakfast and lunch are now free for all NYC public school students, all families still need to complete the School Lunch Form, aka the Family Income Inquiry Form

Woods Community Learning School 335 is a dynamic community hub that provides innovative services to children, parents, educators, and the Crown Heights/Bedford Stuyvesant Community at large. Our goal is to provide academic, social, emotional, health, and mental health services to meet the holistic needs of students, staff, and families in order to strengthen the entire community. We want to remove all barriers to learning by providing a village of support to strengthen student academic achievement and well-being.
